The filing fee is $1500.00, and the forms vary depending on why you are filing and where you file. Do not ever pay for a USCIS form, they are free at uscis.gov/forms. You must use the most current forms, fill them out completely and send them in with all required documents to the Lockbox. They will notify you by email of acceptance or rejection along with a Form, I-797C, Notice of Action, receipt notice indicating they are processing your application.

The date of birth seems to be a point of confusion for people from other countries, and this information must be exact. The United States usually put the month of birth, the day and then the year, while many countries do it in a different order.
